Texas Health Services Authority hosts a multi-disciplinary, vendor agnostic collaborative to remove barriers to safe, secure exchange of clinical information. Inconsistent C-CDA content is impacting transitions of care in the Texas community. The Interoperability Collaborative’s C-CDA Standardization Workgroup consisting of clinicians, IT analysts, EHR vendors and national networks developed a Discharge C-CDA Minimum Dataset. The C-CDA Standardization Workgroup leveraged the experience of clinicians to identify needs, received feedback from a community survey and vetted technology ability with vendor community.
We are requesting that all organizations implement the minimum recommendations for a Discharge Summary C-CDA. We believe it will improve transitions of care.
