What is the requirement?

Admit, Discharge, and Transfer (ADT) notifications for treatment, care coordination, or quality improvement purposes must be sent following Inpatient, Emergency Department, and Observation visits to ensure a ‘reasonable effort’ was made to send electronic notifications to providers, SNFs, and other suppliers at either the provider’s or patient’s request.
